These Terms and Conditions apply to all physiotherapy and cardiac rehabilitation services provided by Surrey Cardiac Rehab. By booking or attending an appointment, you agree to these terms.

1. Professional Standards
Surrey Cardiac Rehab provides services in accordance with:

The standards of practice and professional conduct of the Chartered Society of Physiotherapy (CSP)

The Health and Care Professions Council (HCPC) Standards of Conduct, Performance and Ethics
All treatment is delivered by appropriately qualified and regulated professionals.

2. Appointments
Appointments may be booked by telephone, email, or in person.
By booking an appointment, you agree to:

Provide accurate and up-to-date personal and medical information

Inform us promptly of any changes in your health, medication, or circumstances
Surrey Cardiac Rehab reserves the right to refuse, modify, or discontinue treatment where it is clinically inappropriate or unsafe to proceed.

3. Assessment and Treatment
All patients will undergo an initial assessment prior to treatment. This assessment informs clinical reasoning and the development of an appropriate treatment or rehabilitation programme.
Treatment plans are based on current best evidence, professional judgement, and the information you provide. Programmes may include exercise, education, advice, and lifestyle guidance.
Whilst physiotherapy and cardiac rehabilitation aim to support recovery and improve function, no guarantee can be made regarding outcomes, as individual responses to treatment vary.

4. Consent
In line with CSP guidance:

Informed consent is required before assessment and treatment

Consent may be verbal or written and is an ongoing process

You have the right to ask questions and to withdraw consent at any time
Continuing to attend and participate in sessions is taken as ongoing consent.

5. Your Responsibilities
To support safe and effective care, you agree to:

Disclose all relevant medical information honestly and fully

Follow professional advice and exercise guidance

Inform us immediately if you experience pain, dizziness, shortness of breath, or other concerning symptoms

Take responsibility for activities undertaken outside supervised sessions

6. Fees and Payment
Fees will be clearly communicated in advance of treatment.
Payment terms will be agreed at the time of booking or invoicing. Surrey Cardiac Rehab reserves the right to:

Request payment in advance

Suspend or discontinue services where fees remain unpaid
7. Cancellation Policy
We require a minimum of 24 hours’ notice for cancellations or appointment changes.
Appointments cancelled with less than 24 hours’ notice, or missed without notice, may be charged in full, except in exceptional circumstances.

8. Home Visits
Where treatment is provided in your home, you are responsible for ensuring a safe and suitable environment, including:

Adequate space

Appropriate flooring

Freedom from hazards
Surrey Cardiac Rehab reserves the right to terminate a session if safety cannot be assured.

9. Confidentiality and Data Protection
We are committed to maintaining confidentiality in accordance with:

10. Liability
Surrey Cardiac Rehab holds appropriate professional indemnity and public liability insurance.
We cannot accept responsibility for injury, loss, or damage arising from:

Incomplete or inaccurate information provided by you

Failure to follow professional advice

Activities performed outside supervised sessions
Nothing in these Terms excludes liability for death or personal injury caused by negligence.

11. Chaperones and Support
You may request a chaperone or have a family member, carer, or support person present during sessions.

12. Complaints
We aim to provide high-quality care at all times. If you have concerns, please raise them with us directly so we can address them promptly and appropriately.

13. Changes to These Terms
Surrey Cardiac Rehab reserves the right to update these Terms and Conditions to reflect changes in professional, legal, or regulatory requirements. The most current version will be available on our website.

14. Governing Law
These Terms and Conditions are governed by the laws of England and Wales.
